OUR CARE PLAN PROCESS

Step 1. Intake call- During the intake process we will gather information about your loved one and answer any questions or concerns for the family.

Step 2. In-home assessment- Our in-home assessment is conducted with your loved one's needs, abilities and safety in mind. During this time, we'll gather information about their physical abilities, cognitive functions, daily living activities, and medications. From there a care plan is formed.

Step 3. Meet caregivers- Before the start of care we'll schedule a time for you and your family to meet your caregiver. This meet is to not only ensure that the caregiver is a good fit for your family, but also that your loved one is seeing a familiar face at the start of care.
